Description
This is a copy of the PDF version of the survey that was used for the PRIME Net Hepatitis C Survey. A version of this survey was also first made available in a web-based/electronic form. The target audience was given the option to take the survey electronically or print out this PDF version and either mail or fax back. After several rounds of solicitation, the remaining nonresponders were mailed this paper copy along with a postage paid return envelope. This survey was designed to be machine readable.
